Here are the steps to become a Muslim. You do not need any witness or go to the mosque.You can easily declare by yourself that Allah is the only God and Muhammad is the final Messenger and you can do it anywhere,anytime.
Sahih Bukhari Volume 9, Book 93, Number 469:
Narrated Ibn Abbas:
When the Prophet sent Muadh to Yemen, he said to him, “You are going to a nation from the people of the Scripture, so let the first thing to which you will invite them, be the Tauhid of Allah. If they learn that, tell them that Allah has enjoined on them, five prayers to be offered in one day and one night. And if they pray, tell them that Allah has enjoined on them Zakat of their properties and it is to be taken from the rich among them and given to the poor. And if they agree to that, then take from them Zakat but avoid the best property of the people.”
First of all ask these questions to yourself:
Do you believe Allah is the Only Creator of the whole universe and He Alone should be worshipped and do you accept Muhammad pbuh as the final messenger and slave of Allah?
If YES proceed to the next question
If you were to convert,can you perform the 5 obligatory prayers daily?
If YES proceed to the next question
If you were to convert,can you pay zakah/alms?
If YES
Then do not hesitate to surrender yourself to Allah The Almighty before it is too late.
All you need to do is to pronounce the Shahadah correctly in Arabic then say it again in any language that you can understand so that you know what you said in Arabic.
Below are the steps of pronouncing the Shahadah
- Make intention in your heart that you want to surrender yourself to Allah The Almighty
- Pronounce the Shahadah
Ash-hadu an laa ilaaha illallaah,
wa ash-hadu anna Muhammadan ‘abduhu wa rasuluh.
I testify there is no deity worthy of worship except Allah, and I testify Muhammad is His slave and messenger

3. Take Ghusl/shower
HOW TO PERFORM GHUSL
“The proper method of ghusl (ritual bath) involves the following steps:
Step 1
Make the niyyah (intention) to perform ghusl for purification.
Step 2
Wash your private parts thoroughly with water.
Step 3
Perform wudu’ (ablution) except for washing of your feet, which you can do later while bathing the body.
Step 4
Wash the entire body, starting with your head and the right side, followed by the left.
Step 5
It is preferred that the whole body be washed three times. The minimum is once.Having completed the above, you are considered eligible to perform the salah (prayer).” 
HOW TO PERFORM WUDOO
Step 1
Declare the intention that the act is for the purpose of worship & purity. Start by saying Bismillah (In the name of Allah)
Step 2
Wash the hands up to the wrists, three times
Step 3
Rinse out the mouth with water, three times
Step 4
Cleanse the nostrils by sniffing water into them by the right hands & then remove the water with left hand, three times.
Step 5
Wash the whole face three times with both hands,from the top of the forehead to the bottom of the chin and from ear to ear.
Step 6
Wash the right arm three times up to the elbow,and then do the same with the left arm
Step 7
Wipe the whole head with a wet hand, once
Step 8
Wipe the inner sides of the ears with the forefingers and their outer sides with the thumb. This should be done with wet fingers.
Step 9
Wash the two feet up to the ankles, three times,beginning with the right foot and then the left foot.
After converting make sure you visit a nearby mosque to get a convert certificate.Without this proof you will not be allowed to enter Mekkah for hajj/pilgrimage.
